Notification of Funding Availability for the Direct Services Grant Program from the Healthcare Georgia Foundation


The Healthcare Georgia Foundation has issued a Notification of Funding Availability (NOFA) for the 2019 Direct Services Grant Program. In accordance with the Foundation’s new Strategic Plan, which addresses achieving health equity for all, the Direct Services Grant Program focuses on Preventing and Managing Chronic Diseases and aligns with the Foundation’s Promoting Health and Preventing Disease impact area.

The 2019 Direct Services Grant Program will support new or existing evidence-based, best practice or promising approaches programs that address high burden and costly chronic diseases and underlying risk factors that have proven evidence-based interventions, including:

  • Asthma
  • Cardiovascular disease (heart disease and stroke)
  • Diabetes

Interested applicants are strongly encouraged to participate in the pre-application webinar on July 10, 2019 at 10:00 AM EST. The Foundation staff will discuss the Foundation’s strategic plan, provide an overview of the Direct Services Grant Program, and provide guidance on the evaluation and application requirements.

Online applications for this grantmaking program are due by August 9, 2019 by 3:00 PM EST.
